What this guide helps you do
Use the short New patient form when you need to create a record quickly. It collects only the details needed to identify and contact the patient; clinical, billing, privacy, and other information belongs in the full record.
- Create one patient and open their record
- Add several patients in succession
- Understand which details are required
- Finish the record after creation
Before you begin
- Search the patient list first to avoid creating a duplicate.
- Choose the correct location before opening the form.
- Have the patient’s name and either a phone number or email address available.
Enter the patient’s basic details
Open the form
Choose New patient.
Enter the name
First name and last name are required. Use the patient’s correct identifying name; a preferred name can be added later.
Add date of birth
Enter it when known so staff can distinguish patients with similar names.
Add a contact method
Enter at least one phone number or email address. If you enter an email, it must be valid.
Choose what happens after saving
Create and continue
Choose Create patient to create the patient and continue to their full record.
Add another patient
Choose Save & add another when entering several patients; the form resets for the next person.
Complete the record
In the patient record, add concession and referral details, custom fields, communication choices, address, emergency contact, and any other information your clinic requires.
Things worth knowing
- A duplicate record can divide appointments, notes, invoices, and communications between two places. Search first.
- Do not put clinical information into name or contact fields.
- Creation is only the first step; verify privacy and communication choices with the patient.
