About your intake form
Before your visit, your clinic may send you a short link to fill out an intake form. It's a simple, secure way to share some background with your care team ahead of time, so they're ready for you the moment you walk in — no account or sign-in needed, just the link you were sent.

What you might see on the form
Every clinic's form is a little different, but you'll generally see a mix of the following question types.
- Short answer — a simple text box for brief responses.
- Multiple choice — pick one option from a list.
- Checkboxes — pick one or more options that apply to you.
- Dates — for things like your date of birth or when symptoms started.
- File uploads — if your clinic asks you to attach something, like a photo or a document.
- A signature box — draw or type your signature where needed.
- A body diagram — sometimes used so you (or your clinic) can mark specific areas, like where you're feeling pain.
Filling it out at your own pace
As you answer each question, your progress is saved along the way — so if you need to step away and come back, you generally won't lose what you've already entered. Take your time, and answer as best you can. If a question doesn't apply to you, you can usually move past it.
